How's Your Health

Please do a How's Your Health questionnaire at least once a year.  If you are already my patient, or are scheduling your first visit, PLEASE ENTER MY CODE: IMP402 when you start the survey and please request that the information be emailed to me at the end of the survey. That way, I will get valuable information from the process. If you are not my patient, I encourage you to do the questionnaire anyway. Do not enter a code, but print it out and bring in to your current physician. They will probably find it helpful. THANK YOU!

Virtual visits

I am now also offering virtual visits (visits done by email or phone) for established patients with very stable conditions that need routine follow up or for those who have relatively simple problems that don't need face to face contact. If you are interested in this type of visit, click here.
